Emotional Support Animal

Students requesting an Emotional Support Animal (ESA) as a housing accommodation must be registered with the Student Accessibility Services office. No housing or ESA accommodation requests will be reviewed from August 1 – September 1.

Please note: all animals must have approval prior to their arrival to campus.

 

Disability Documentation Requirements

Documentation is required for all accommodations. Your provider may use聽THIS FORM or write a letter that meets the required criteria below.

Documents provided must:

  • Be on professional letterhead
  • Denote absence/presence of disability
  • Include the聽impact of the disability on daily activities
  • Explain how the request may lessen or alleviate the impacts to daily activities
  • Provider signature
